New Patient Information



We welcome you as a new patient!
Here is some information you should know before coming to the office:


During your first office visit, you will see Dr. Psonak who will review your medical history, do a physical exam appropriate for your condition, and formulate an initial treatment plan. At that time he may recommend specific laboratory testing or other diagnostic procedures. Due to the nature of Integrative Medicine, you are not "rushed" through the office.  Dr. Psonak recognizes that everyone is a unique "bio-individual" with individualized needs. You are encouraged to ask questions, and take notes. Please allow time in your schedule so you will not feel rushed, as Dr. Psonak is very thorough and does not "rush" through the visit. Plan on spending about 1 1/2 to 2 hours at your initial visit, depending on the complexity of your medical concerns.  A follow up office visit to refine the treatment plan, recommend supplements, lifestyle changes, dietary modifications and review any laboratory results will usually be scheduled 1-2 weeks later. If a test for heavy metals is indicated, a provocative IV may be given at the initial visit, at which time you will start collecting urine, continuing at home for 6 hours, sending a sample to the lab the next business day.  


  1. Ask your current health care providers immediately to mail, e-mail or Fax a copy of your medical records to the address/Fax# listed below. It is very important that your records be available in our office at the time of your first visit.
  2. Bring a complete list of medications and supplements you are taking.
  3. Download a New Patient Health questionnaire, print it out, and complete it before your visit. If you need a questionnaire mailed or faxed to you, please call the office.
  4. This is a fragrance free office for our chemically sensitive patients. Any patients wearing perfume, after-shave, fragrances or residue of smoke on their clothes, will be asked to leave and another appointment will be set for them.
  5. Please ask if you should come in fasting for your initial visit, particularly if you have not had any laboratory blood work performed in the last two months.
