New Patient Information

We welcome you as a new patient!
Here is some information you should know before coming to the office:

WHAT TO EXPECT:
During your first office visit, you will see the doctor who will review your medical history, and do a physical exam appropriate for your condition. Please ask your current health care providers immediately to mail or FAX a copy of your medical records to the address/Fax# listed below. It is very important that your records be available in our office at the time of your first visit. Also, bring a complete list of medications and supplements you are taking. Please come in 15 minutes early to fill out a questionaire in the office, or download one to fill out before you come in. Most initial visits take about two hours and cost $300.00. Lab costs are separate. If a test for heavy metals is indicated, a provocative IV may be given, at which time you will collect urine at home for 6 hours, sending a sample the next day to the lab.

IMPORTANT:

This is a fragrance free office for our chemically sensitive patients. Any patients wearing perfume, after-shave, fragrances or residue of smoke on their clothes, will be asked to leave and another appointment will be set for them. Please ask if you should come in fasting for your visit.
